This page includes information on how to request reports and refer a patient to the Radiology department at Great Ormond Street Hospital (GOSH).
Radiology Imaging: seek a second opinion
To request a second opinion on Radiological Images, external teams need to complete the form below and send it to the Radiology department at GOSH or email it to Gos-tr.externalimages@nhs.net.
Please note that your trust will be charged for this service. GOSH teams can submit a request via the EPR system.

Please note that external second opinion requests are not covered by the on-call service Radiology provides for GOSH patient related cases.

GOSH Imaging transfers and Reports
For imaging performed here at GOSH to be transferred to other NHS providers please
contact your Radiology department. Requests to send or receive imaging are done via IEP.
For queries regarding that transfer, for example if IEP is not working, please email
external.images@gosh.nhs.uk.
Reports are sent separately, hence NHS providers need to email
gos-tr.externalimages@nhs.net.
Please clarify what imaging report you require, unless you require all.

For families to request patient records please contact Records.SAR@gosh.nhs.uk.
If your child is patient at GOSH please consider downloading and using the GOSH app (MyChart - MyGosh).
