Transitioning to adult services workbook

The Young People‘s Forum (YPF) created the below booklet with HATO and GOSH Arts during a number of workshops to help make the transition from children and young people services to adolescent or adult services easier for patients. Some YPF members are currently going through transition and have shared their thoughts and feelings as well as top tips about transition in this workbook. We really hope that patients will find all the advice and the suggested activities helpful.
