Independent report released on Peter Connelly paediatrician following new legal advice

Great Ormond Street Hospital today releases an independent report we commissioned into the practice of Dr Sabah Al-Zayyat, the locum consultant paediatrician who saw Peter Connelly in Haringey shortly before he died.

The Trust believes that most of the information in the report has been in the public domain for some time, much of it for some years.

Great Ormond Street Hospital reiterates the public apology we made in November 2008 regarding Peter’s tragic death.  We’re truly sorry Peter suffered and died.  Looking after vulnerable children is the most important thing we do.  Since the death we have worked hard with all our partners to seek to strengthen services in Haringey, and this has been recognized by independent assessments such as the recent OFSTED and CQC reports.

The Trust had been previously unable to publish the Dr Al-Zayyat report, (May 2008) on legal grounds.  New advice permits substantial parts of the report to be released.  We can also release the specific submission made to the first serious case review.  The submission consists of an additional document based on the report and the action plan at that time (25.05.08)

The Trust took over responsibility for the service in April 2008 and received the report a month later, in May 2008. The report demonstrated that the Trust needed to act and we did.  We acted both in terms of Dr Al-Zayyat’s employment and professional registration, and the report vindicated decisions we had already taken to strengthen the service.

 

Update January 2012

As further information enters the public domain, the Trust has been able to review what can be released.  We are now able to publish below: the report; the extract from the report and the action plan shared with the Chair of the LSCB on 12th May 2008; and the formal submission to the Serious Case Review and the new action plan shared on 28th May 2008.
